AGE WAIVER REQUEST CHECKLIST

The following items must be completed and sent to the GED Testing Office in order to be considered for an age waiver. Failure to include any of these items will delay the application process and the age waiver will be denied.

1. ___________ GED Age Waiver Application completely filled and notarized.

2. ___________ Copy of student’s official withdrawal from high school (May be obtained from Guidance Office).

3. ___________ Copy of student’s scores from the official GED Practice Test. Total score must be a minimum of 2500 with a minimum score of 500 points on each subtest.

4. ___________ Letter written by student (notarized) reason for the age waiver request.

5. ___________ Letter written by parent (notarized) granting permission to take the GED Test Series and if necessary, stating reason for age waiver.

6. ___________ Documentation of reason checked.
